In 1938 he worked as an assistant pharmacist to Professor S. de Boer in Groningen University. He completed his undergraduate general medical studies in Groningen in 1941. In 1941 he travelled to Newcastle on Tyne to Durham University hospital where he spent three months in an academic hospital in the fields of paediatrics, surgery, obstetrics and optometry. He then spent a further 1 1/2 months in neurology before returning to Groningen. Johann received his doctorate in medicine in Kiel in 1943, and also met his wife (Grete Frahm) while there. From April 1942 to October 1943 he worked as a war auxiliary physician (Kriegshilfarzt) in the Universitäts Kinderklinik in Kiel. In November 1943 he joined the Kinderklinik in the Reichsuniversität Straßburg in the role of Wissenschaftliche Assistent, and this contract was until 1945. He did not formally join any political party and claimed to be apolitical, although he was a member of the Germanic SS from 1942 (a political and paramilitary organisations established in occupied North West Europe under the supervision of the SS). In 1945 the Clinique Infantile in Strasbourg declared him not ideologically affiliated with Nazism and recommended that he continue to be employed as a paediatrician in Stephansfeld.
